Final
Report: Instructions
The last
step in the SSJCF grantmaking process is the completion of a Final Report. It is
intended that this evaluation serve not only stewardship and accountability
purposes, but that it be a learning experience for both you and the Charitable
Fund. We also encourage you to include your suggestions and comments regarding
the evaluation and other aspects of the grants program.
FYI:
Included in the grant award packet will be a "hard copy" of these
reporting guidelines; a deadline for submission of these reports will be
specified (generally, within three weeks upon completion of the project).
Please
respond to each section (and sub-section) as completely as possible, keep within
the page limitations as indicated, and follow the format guidelines listed
below.

Achievement of Objectives (Not to exceed two pages) |
- The original grant
proposed several objectives for the project. Please restate those objectives
and indicate what was accomplished for each.
- Given those objectives
and level of accomplishment, rate the level of goal attainment for
the overall grant project/program (low, medium, high) and the
rationale for the rating.
- Briefly describe any
factors which created difficulties in accomplishing the objectives.
