Project & Program
Guidelines
The SSJCF seeks to support the projects and/or programs
of eligible agencies that promote
health and wellness and are within one of the Fund's areas of
focus.
Factors
to consider when requesting funds for a project or program include:

What is the health/community need your
project/program will address; and, how was this need determined? |
|
|
What are the goals and strategic objectives of the
project; and, what activities will be taken to carry out those objectives? |
|
|
What will be the role of other, collaborating groups
in implementing the project? |
|
Why is your organization well-positioned to implement
this project; and, how does the project relate to your organization's mission
and goals? |
|
How will the proposed activities benefit the
community in which they occur; and, how will success/progress be evaluated? |
|
What steps have been taken to secure additional funds
to sustain the project? |
